Flood Claim Form
Public Act 222 of 2001 provides procedures that need to be followed in the event of a sewage overflow or back up on your property.  Any citizen that experienced a overflow or back up due to the severe weather on 8/11/14 must file a written claim with the City of Hazel Park within 45 days after the overflow or back up occurred.  The City urges you to print, fill out and submit the claim form to the City Clerk prior to 9/25/14.  Please click on the "Read More" link below to be directed to flood information page to view and print the claim form, or please call the City Clerk with any questions.
